What is the root apex?

In dentistry, apex usually means the tip of a tooth root. A root may curve; its outer tip and the root canal opening do not necessarily coincide exactly. The term alone does not describe a disease.

The root end

The external shape defines the anatomical apex. A multi-rooted tooth has a separate root-end region for each root.

Opening is not tip

Blood vessels and nerves pass through openings near the root end. These may be offset to one side and have branches.

Why it matters

Root canal treatment requires an individual working length. The visible radiographic root tip is not automatically the correct endpoint for every instrument.

What is explained?

The root-end region.

Why distinguish the structures?

Tip, canal opening and working endpoint can differ.

Often confused

Apex or apical foramen?

The apex is the external root tip. The foramen is an opening of the canal system near that region.

Quick answers
Does apical always mean inflamed?

No. Apical describes location towards the root tip. Additional findings are needed to describe disease.

Is the radiographic tip an exact anatomical measurement?

A two-dimensional projection may differ from actual anatomy. It is combined with other findings.

Does every root-end finding need surgery?

No. Cause, previous treatment and restorability determine the options.

What do we see at the root end, and what other findings are needed?

A report mentions the root apex? Ask what the location and finding mean for the tooth.
