Why can the tongue look hairy or dark?

In hairy tongue, the thread-like papillae on the upper surface become elongated and more keratinised. These are not real hairs. The surface may look dark, brown or another colour; colour alone identifies neither the cause nor a fungal infection.

Papillae, not hairs

Reduced shedding of surface cells can allow thread-like papillae to elongate.

02 / 03

Consider contributing factors

Smoking, some medicines, coffee or tea and changes in oral care can contribute. Not every dark coating is hairy tongue.

03 / 03

Use gentle care

Assess possible causes and clean the tongue gently. Do not stop medicines on your own; persistent or painful changes need examination.

Often confused

Hairy tongue and oral thrush

Elongated papillae do not automatically mean a fungal infection. Colour and a photograph are insufficient to distinguish them.

Quick answers
Is a black tongue dangerous?

Confirmed hairy tongue is usually benign. A new dark area cannot reliably be classified online and needs assessment if uncertain.

Do I need an antifungal?

Not automatically. Hairy tongue and oral thrush are different findings; treatment should match the established cause.

Will scraping remove everything?

Gentle cleaning may help. Harsh scraping can irritate tissue, and symptoms or coating may have another cause.

For a personal discussion

Editorially suggested question to discuss with Dr Niusha Barahmand Pour:

Which factors could alter the papillae, and which care is gentle?

Persistent unexplained coating or burning needs examination rather than comparison with online pictures.
