Tooth pain after a filling: what matters now

A tooth can be temporarily sensitive after a filling. Severe, worsening or persistent pain needs assessment, however. The trigger, duration and course matter, as do swelling or other symptoms. A single symptom cannot reliably identify the cause.

What kind of response occurs?

The new filling and treatment of the tooth may cause temporary irritation. A brief response to cold or warmth may settle. Whether observation is appropriate depends on the findings and the individual aftercare advice. A single waiting period cannot cover every situation.

Contact the treating practice if symptoms do not clearly improve, appear for the first time or get worse. Pain without an external trigger, pain that wakes you at night or pain that lingers after a temperature stimulus warrants prompt assessment. A recent filling does not automatically explain every new symptom.

Does it hurt particularly when biting?

If the treated tooth feels too high when you close your mouth or seems to meet first, the practice should check the bite. A disturbing contact may be adjustable. Do not file the filling yourself or repeatedly test the painful tooth by biting hard.

A high contact is only one possible explanation. Other problems with the tooth can also cause chewing pain. “The filling is too high” is therefore not a reliable self-diagnosis, and pain on biting does not automatically mean root canal treatment is necessary.

When should it not wait?

Severe or rapidly worsening pain, swollen gums or face, or fever requires urgent dental assessment. A new, severe restriction in mouth opening, particularly with swelling or fever, needs immediate medical or dental acute assessment. Contact a practice, the emergency dental service or urgent medical care; do not wait for a routine appointment.

Breathing or swallowing difficulty, extensive spreading swelling or swelling involving the eye area requires immediate medical help. In Germany, call 112 for a life-threatening emergency. Do not wait for a routine check-up.

What helps until the check?

While the anaesthetic is still working, you can injure your cheek or tongue without noticing. Follow the practice’s eating and drinking advice, especially avoiding chewing on the numb side and hot drinks. Soft foods and avoiding temperatures that trigger sensitivity may be more comfortable temporarily.

Continue oral hygiene gently rather than scrubbing at the filling. Follow the practice’s individual aftercare instructions. Ask the practice or a pharmacist about pain medicines, taking existing conditions and other medicines into account. Pain relief does not replace assessment of the cause.

Four details help at the next appointment

The practice can examine the filling, the surrounding tissues and the bite. The findings determine whether further investigations, an adjustment or another treatment is appropriate. These observations help make the discussion specific:

  1. When did the symptoms start: immediately after treatment or later?
  2. What triggers them: cold, warmth, chewing or no apparent stimulus?
  3. Does the pain settle quickly or linger, and is it improving or worsening?
  4. Is there also swelling, fever, a changed taste or difficulty opening the mouth?

Common questions

How long is pain after a filling normal?

A fixed time limit is not a reliable guide. Mild, brief sensitivity that is improving can be temporary. Contact the practice if it is not improving or pain is persistent or worsening; warning signs require urgent assessment.

Will the tooth need root canal treatment?

Not automatically. Temporary irritation or a disturbing bite contact are among the possible explanations. An examination is needed to assess whether the tissue inside the tooth requires treatment.

Can a filling be too high?

Yes, a disturbing contact can cause symptoms. If the tooth seems to meet first when biting, the practice should check the bite. That sensation alone does not establish the cause.

What if the pain starts later?

New symptoms after an initially comfortable period warrant another examination. Do not simply wait because the tooth already has a filling. Timing alone cannot provide a reliable diagnosis.
